Tutor Reporting Forms

One-on-One Reporting

Each quarter, BL and ESL one-on-one tutors are asked to submit a report of their tutoring activity for the prior three months. These reports can be submitted electronically, or printed and then mailed to our office. Quarterly updates are very important because they provide Washtenaw Literacy with the information we need to better serve our learners and tutors, apply for grants, and keep our files up to date. Each quarterly update will collect slightly different information.

The reporting schedule is as follows:

June 30	March 1 – May 31	Tutor satisfaction survey

Tutor Survey March-May 2013 will be available beginning May 28, 2013

Group Reporting

Twice a year, BL and ESL group tutors and learners will be asked to give feedback about their group tutoring experience. This information will be collected during a scheduled group meeting time.
